How old must my pet be at enrollment? [ click here ]
Your pet must be at least 8 weeks old to enroll.
What do I have to do to keep my pet's insurance in good standing? [ click here ]
Ensure that your pet has an annual physical and keep up to date with the vaccinations recommended by your veterinarian.
For pet owners who choose to purchase Companion Complete, we request that you ask your veterinarian to forward your pet's medical records to us within 30 days of enrollment (failure to do so may result in cancellation).
What veterinary costs are covered? [ click here ]
Veterinary fees covered include, but are not limited to: prescribed medication, x-rays, surgeries, hospitalization, ultra-sounds, MRI/CAT scans, homeopathic treatments including acupuncture and chiropractic, chemotherapy and referrals. Will hereditary conditions be covered?[ click here ]
Yes. Hereditary conditions will be covered unless the problem was noted, treated, symptomatic or diagnosed prior to enrollment.
Are there any veterinary costs that will not be covered? [ click here ]
Coverage is not provided for pre-existing accidents and/or illnesses or for elective surgery such as tail docking or de-clawing
What is a deductible? [ click here ]
The Deductible is the stated dollar amount per accident, or Illness Condition for which the Insured is responsible.
What is a Co-Payment? [ click here ]
The Insurer will reimburse a specified portion, seventy to ninty (70-90) percent, of each covered claim up to the Insured Limit. The Insured’s share will be the residual amount, ten to thirty(10-30) percent, of each covered claim. This is referred to as the co-payment. Once the Insurer’s share of each covered claim has been established the applicable deductible will be applied. The net will be reimbursed to the Insured.
I have heard that some breeds of dog are more prone to hereditary conditions. [ click here ]
Yes. If you enroll your dog into the Companion Complete program, hereditary conditions will be covered unless the problem was symptomatic or diagnosed prior to enrollment.

A $2 fee for monthly pay plan will automatically be included each month in your monthly billing.

You will receive a claim form with your document of insurance. You may also download our claims form here
or call us at 1-866-600-0501.
Simply fill in the Owner portion of the form and ask your veterinarian to fill in the Veterinarian portion (part 2) of the claim form. Send that form back to us with the paid invoice and details of treatment, including medical records. Claims cannot be processed without a claim form completed by you and your vet, the paid invoice and related medical records. Fill out a claim form.
